Recording A Witness For A Destruction Or Return To Sponsor

After an item is marked as destroyed or returned to the sponsor, another user can use the witness function on inventory that you have destroyed or sent back to sponsor. Some sites may require this at the drug build level, or it can be optionally completed. 

If someone else edits the Destruction or Return to Sponsor action, the original user who performed the destruction or return, can then witness it.

From Facility Alerts

Items will only be visible in the facility alerts if the drug build is set to required a witness.

1. In the facility Alerts, click on Drug Returns
2. Select the alert for Witness for Destruction or Witness for Return to Sponsor
3. Locate the Item you want to witness and click Resolve
   

   Tip - If there are no items listed, that means you were the original user to record the action. You cannot witness your own destruction or return to sponsor.

    

4. Check all items to record a witness for
5. (Optional) Adjust the Witness Service Date
6. (Optional) Enter any Witness Comments
7. Click the blue Submit button

At the Protocol

1. Navigate to the Protocol
2. Click on the Inventory Tab
3. Click on Manage Drug Returns
4. Click on the correct Disposition tab
   1. Items Already Destroyed
   2. Items Sent Back to Sponsor
5. Scroll down and click Record a Witness
6. Check all items to record a witness for
   

   Tip - If there are no items listed, that means you were the original user to record the action. You cannot witness your own destruction or return to sponsor.

    

7. (Optional) Adjust the Witness Service Date
8. (Optional) Enter any Witness Comments
9. Click the blue Submit button
