Submitting the Annual Residency Accreditation Report
Instructions for RPDs to submit the Annual Residency Accreditation Report in PharmAcademic.

The Annual Residency Accreditation Report is a required annual survey completed in PharmAcademic. It captures key program data for ASHP to support accreditation, monitor program performance, and identify trends across the pharmacy community.
This task can be completed by the Residency Program Director (RPD) or Interim RPD (if assigned).
Important: Only the RPD or Interim RPD can submit the report. If reassignment is needed, contact the PharmAcademic Support Team.
Before You Begin

Ensure all residents from the prior residency year have been closed out in PharmAcademic.
- On several pages of the Report, the displayed data is pulled from PharmAcademic. If the data is incorrect, you will be asked to exit the report, update your program in PharmAcademic, and refresh the report data.
Overall Instructions
1. Access the Report on your global task list on the PharmAcademic Home page.
2. Review the Introduction page and click the Next button to begin the survey. Key information:
- Due date.
- Contact information for support: For accreditation-related questions, contact asd@ashp.org. For system support, visit the Help Center or contact the PharmAcademic Support Team.
3. Close Out Residents (If needed).
Review the table listing residents with end dates in the past who have not been closed out of PharmAcademic. If any residents need to be closed out, exit the report, complete the close-out process, and return to the report to continue.
4. Complete each page of the Report.
- Use the progress bar to track completion.
- Use the page dropdown or navigation buttons to move between sections.
- Enter or review data on each page as required. Please see the questions below.
Note: Your responses are saved when you click the Next, Previous, or Close buttons. Closing your browser without using these options will not save your work.
5. Submit the Report.
- On the final page, click the attestation checkbox, “I attest that this survey has been completed accurately for this residency program.”
- Verify your name is displayed correctly.
- Click the Submit button. Note: If you have any unanswered questions, PharmAcademic will direct you to the page of the survey to provide a response.
Important: Once submitted, the report cannot be edited. You will only be able to view or print it.
6. Access the submitted Report.
- Click the Dashboards button.
- Select the Program tab.
- Click Annual Residency Accreditation Reports.
- Click the View link.
- Click the Print/Save link, if needed.
Questions & Guidance
Page 1: Residency Positions
Page 1 of the Annual Residency Accreditation Report asks about the number of residency positions for the current residency year.
On this page:
- Enter the number of residency positions for the current year for your accreditation code (including all NMS codes). PharmAcademic will automatically calculate “the changes from last year.”
- Enter the number of total applicants you had for the position(s).
Note: Residents in PGY1/2 combined programs should be included in the PGY2 program’s count for both the first and second year.
Page 2: Residents who did not successfully complete program
Page 2 will display any residents from the previous residency year that did not successfully complete the program.
This information is pulled directly from PharmAcademic when you closed out your resident and is read-only. If the information listed is not correct, please contact the PharmAcademic Support Team.
Page 3: Positions For Residents who completed the program
Page 3 will display residents who completed the program in the previous year. It will indicate:
- Whether the resident was hired into a position requiring their residency training
- Whether the organization providing the training hired the resident
This information is pulled directly from PharmAcademic when you closed out your residents. If the information is not correct, please exit the report, update the data in PharmAcademic, return to this page, and click the Refresh Current Year button.
Instructions for Updating Closed out Residents Information

Exit the report.

Go to the Residents tab.

Click the button View Closed Out Residents.

Click on the resident’s name.

Click the Edit link on the Program tab.

Edit the resident’s information.

Click the Save Program Edit button.

Return to the report and click the Refresh Current Year button on page 3.
Page 4: number of preceptors
Page 4 of the report displays the total number of preceptors in your program, along with the number of preceptors assigned to required, elective, and selective learning experiences. The report also compares the current data with data from the previous year to help identify changes over time. Additionally, this page includes a question asking whether your program has a sufficient number of preceptors to support resident training.
This information is pulled directly from PharmAcademic and is read-only. If the information is not correct, please exit the report, update the data in PharmAcademic, return to this page, and click the Refresh Current Year button. Please see the instructions below on updating your learning experiences.
Instructions for Updating Learning Experiences
You can view if the learning experiences are marked as “Required,” “Elective,” or "Selective" on the Learning Experience tab and/or by clicking “Download List” to run a report of your learning experiences.
- Exit the report.
- Go to the Learning Experience tab and click View next to the learning experience.
- Click the Edit Learning Experience button.
- Select the radio button next to Required or Elective.
- Click the Save button.
- Return to page 4 of the report and click the Refresh Current Year button.
Page 5: Longitudinal Projects
Page 5 asks the number of residents who longitudinal/residency projects resulted in an institutional change in policy, service, workflow, etc.
Page 6: Residency Program Updates
Page 6 asks about organizational changes and any licensure or accreditation changes. If you answer yes, you will be required to provide additional details in the text box.
